Use the family editor to make a customized titleblock layout with labels and a revision table.

Revit title blocks are families. Use the family editor to create a custom title block for use in your projects.
Begin from Revit Home and click New to create a new family. In the new family dialog, navigate to the title blocks folder. Select the size for your new title block. In this example, we will create a title block for an A0 sized sheet. It is also possible to create a custom sized sheet.
The lines in the template represent the extents of the paper. The custom title block geometry is created within this boundary. On the create tab, select the line tool on the contextual tab and select the subcategory of the line. In this case, a medium line will be used to identify the main presentation area of the title block. Additional lines are added as required.
On the insert tab, click image to import a logo to the title block. On the create tab, click text to add text to the title block. Use the type selector to change the size of the text created. Text is used for information that is not going to change from project to project, such as the titles of the different areas in the title block.
Use labels for information you want to connect to sheet parameters. Labels are changed by changing parameters assigned to the sheet element in the project. When you place the label, select the sheet parameter it will be connected to. If required, you can also create custom parameters for the title block information. In this example sheet number is selected for the label. Add labels for other information as required.
On the view tab, create a revision schedule that includes the number, description, and date of the revisions. Edit the schedule appearance of the schedule to match your standards. Then in the title block view, drag the revision schedule from the Project Browser to the sheet.
Once the title block family is complete, load it into your project and create new sheets with the family. Use the parameters from the sheet to fill out the title block information as required.
