D3 - Implement containment actions

Depending on the severity of the issue, you will likely need to define and implement containment actions to isolate the problem for continuing to happen further. Fill out all relevant information in the D3 – Implementing Containment Actions section in the main 8D Report tab.

1. Click Edit.

2. In the D3 – Implementing Containment Actions section, enter the following:

   • Containment Action Description - A description of the action taken.

   • Est. Effectiveness (%) - An estimation of the effectiveness of the action.

   • Results - The result of the action.

     Note: At least one action is required before the workflow can proceed.

3. Repeat for as many actions that are required.

4. In the Containment Actions field, click to select the category the containment actions fall into.

5. In the Containment Actions Summary field, enter additional information regarding the corrective actions.

6. Click Save.

Once all actions have been documented and implemented, you can proceed to the next phase.

• Click the Return transition if further information needs to be added to the problem description.
• Click the Cancel Report transition if no further work is required. This is the last phase from which you can cancel the 8D report.
• Click the Proceed to D4 Root Cause Analysis transition to move to the D4 phase.
