Create a Form Template from a Smart PDF

You can upload a smart PDF to create a template in the Forms tool. Smart PDFs contain dynamic fillable fields, which contributors fill out in the Forms tool directly. A smart PDF can be created in a PDF creator tool like Adobe or Bluebeam.

Upload a Smart PDF into a Form Template

  1. Navigate to the "Templates" tab in the Forms tool, click Create, then select "Create template".

  2. Select "Upload an existing PDF form" and click Next.

  3. Drag PDF files into the window or click From your computer to select one or more PDFs to upload. When the upload is complete, click Next.

  4. Edit the form name.

  5. Select a template type. You can later filter forms by template type, which is useful when your project has many forms.

  6. Choose whether you want to include weather data or a notes field.

  7. Click Next.

  8. Select a model for how contributors will use and submit forms created from your template and click Next. Learn more about each collaboration model here.

  9. Select editors, reviewers, and viewers and toggle on any options.
